Personal and Audiological information

By accpeting the Terms and Conditions, the user hereby, permits Moolman & Associates to send my personal information, as needed, for the processing of claims by medical aids in conjunction with medical aids. Furthermore, should results indicate that the user needs further medical attention, by accepting the terms and conditions, the user gives permission to Moolman & Associates to make use of the user’ personal information and results for all necessary communications with other medical institutions and relevant medical professionals.

Accounts

All accounts will be sent to the user’s medical aid for payment. However, if the users’ medical aid does not pay the account, the user will be held responsible for the settlement thereof. If the user is a private patient, the user is responsible for settling the account on the day of consultation or hearing device fitting. Please note: That if the payment is not made within 90 days, your account will be handed over to our attorneys for collection. The user will be held liable to pay any collection costs and/or attorney fees on the client/attorney scale. Please note: That a R50 administration fee is payable with all first consultations at this practice.

CONSENT CLAUSE IN TERMS OF THE PROTECTION OF PERSONAL INFORMATION ACT, 4 OF 2013

In line with the Protection of Personal Information Act, 4 of 2013 (POPIA), the user acknowledges that the user has been adequately informed of the user’ rights and the purposes for which the user Personal Information will be processed by Moolman & Associates, and that the user has read and accept the terms of Moolman & Associates’s Privacy Policy which can be sourced on their website (“the Privacy Policy”), as well as any privacy notice to which this acknowledgement relates (“Privacy Notice”), whether such Privacy Notice forms part of this correspondence with Medical Aid on behalf of Patient/client to ensure Medical Aid Payment/s. Moolman & Associates’s Privacy Policy or comprises of an independent Privacy Notice can be viewed on/at Moolman & Associates’s Website(s). Accordingly, the user gives consent to Moolman & Associates and approved third parties to lawfully collect, process, retain and distribute relevant Personal Information where it is required by Moolman & Associates for the purposes set forth in their Privacy Policy and more specifically in order for Moolman & Associates to comply with its obligations towards the user and for the purposes of correspondence with Medical Aid on behalf of Patient/client to ensure Medical Aid Payment/s, statutory compliance, statistical analysis, as well as to direct market additional products and services to the user by means of electronic communications, in the users’ capacity as a client of Moolman & Associates.

The user understands the users’ right to privacy and the right to, upon request, have my/our Personal Information reviewed and to object to Moolman & Associates processing of my/our Personal Information. In relation to any Personal Information pertaining to any third party/ies such as our Patients/Clients in respect of which I/We are a competent person or authorised, I/We declare that I/We am/are in fact duly authorised to share any Personal Information as contemplated in this Agreement with Moolman & Associates and consent to the processing thereof for the purpose(s) set forth in Moolman & Associates’s Privacy Policy and/or Privacy Notice
